The Tissue Viability service covers patients in both primary and secondary care setting within the CCICP area, including nursing home’s, patient’s home, and special schools. 

The Tissue Viability team offer a service to all clinicians on prevention of wounds and the management of complex wounds. 

The patient will be seen in whichever environment is most appropriate.  This can be in the patients own home, nursing home, wound care clinic, GP surgery, in hospital or as an outpatient. 

Patients will be seen with the referring clinician; an assessment will be undertaken and plan of care agreed. 

Our team consists of a Clinical Lead, Tissue Viability nurses and a administration team. 

How to Refer

Patients can be referred to the service by their GP, or by any other health or social care professional.
