The Integrated Placement of Care Hub (IPOCH) is a team that works to ensure the safe & timely discharge of patients from hospital.

The service focuses on a home first approach; one of rehabilitation and the promotion of independence for patients leaving acute/bed based settings, or requiring step up care in their own homes. 

The service also provides end of life care for patients under the Palliative Care in Partnership service.

IPOCH is made up of Nurses, Assistant Practitioners, Trainers, General Nursing Assistants and Administrative Co-ordinators who are co-located at Leighton Hospital.

The team works collaboratively to ensure the safe and timely discharge of patients from hospital under Mid Cheshire Hospitals NHS Foundation Trust. The service provides a home first approach with NHS packages of care delivered to people within their own homes, this allows patients who require support following discharge to recieve care in their own home. 

The IPOCH service also supports patients who require short-term community bed-based rehabilitation or recovery.

The service is available to adults aged 18 and over, who require additional support in order to be discharged from hospital settings, or to avoid admission.

 

Patients can be reffered to the IPOCH service via hospital staff, community hubs, GPs and social services.

The Integrated Placement of Care Hub (IPOCH) is excited to have the opportunity to expand its team of General Nursing Assistants (GNAs). The aim of the GNA role is to provide high quality care to patients within their own homes.

The IPOCH service uses a home first approach, one of rehabilitation and promotion of independence for patients leaving bed-based/acute settings or requiring care in their own homes.

If you have experience of working in care and feel confident driving across the Mid-Cheshire area, the team would love to hear from you! Full training is available along with plenty of support from the wider team. The service operates 7 days a week from the hours of 8am – 10pm.
