The Complex Care Service involves the caring of children and young adults requiring various types of clinical intervention. Bespoke care packages are allocated after an assessment has been completed, the child has met the criteria, and funding has been agreed. 

The aim of the service is to support children and young adults with long term complex health needs at home by providing care to optimise the highest quality of life and independence. The service also aims to treat children and young people in the community for as long as possible to avoid hospital admissions.

Our departmental objectives

To support children and young adults with long term conditions who live at home. We aim to treat patients as long as possible without hospital admissions.

Team Leaders manage the packages of care and Nurse Trainers train and educate Healthcare Assistants (HCA) using competencies and training tools. When competent, the HCAs deliver care in the home environment supporting both the child and the family.

Monday - Friday, 09:00 – 17:00

Referrals are received via health professionals or social workers.
