Now that your treatment is complete your colorectal surgeon and Colorectal CNS would have signposted you to your personalised follow-up pathway.

Your colorectal CNS will have given you a copy of your risk stratified follow-up from one of the options below.

This is an example of all the surveillance follow-up offered. However remember this is dependant on your histology results.  

Cheshire Bowel Cancer Screening

Everyone aged 60 to 74 who is registered with a GP and lives in England is automatically sent a bowel cancer screening kit every 2 years. The programme includes some 56 year olds.

Make sure your GP practice has your correct address so your kit is posted to the right place.

If you're 75 or over, you can ask for a kit every 2 years by phoning the free bowel cancer screening helpline on 0800 707 60 60

What is Personalised Care?

  • Personalised Care is a new type of follow up at Mid Cheshire Hospital Foundation Trust.

  • It means you work with, and are supported by your healthcare team to return  to a lifestyle you enjoyed before treatment.

  • We will advise you of important signs and symptoms to look out for.

    • Your test results will be tracked and reviewed by the appropriate clinician in a virtual clinic depending on your pathway, some patients will still need a face to face appointment with a consultant.

  • It does not mean you're alone, you will still be able to contact your Colorectal Support Team at any time during your Personalised Care follow up.

  • Your Colorectal Support Team will be there for each individual and their personalised care needs.

We now hold a Colorectal workshop as part as the Colorectal Cancer Surveillance. The workshop is held once a month and all newly diagnosed, post op patients are invited to attend.

The workshop is run by a Nurse Specialist and a Support Worker who will run through a presentation regarding your Follow up and all signs and symptoms too look out for.

At the workshop you will be informed of how to look after yourself and what is to be expected regarding your surveillance for the following 4 years.

We have received favourable feedback but continue to make changes to the presentation from patient suggestions.
