Transitioning from Children’s to Adult Diabetes Care
Moving from children’s to adult diabetes care is an important milestone, and we’re here to make the transition as smooth and supportive as possible.
If you were diagnosed with diabetes in childhood—most often Type 1 diabetes—you’ll have been cared for by the Paediatric Diabetes Service. When you turn 18, your care will transfer to the Adult Diabetes Specialist Service — that’s us.
Once we receive your referral from the paediatric team, we’ll send you an invitation by text or letter. From that point, you’ll have open access to our service until the age of 25. This gives you plenty of time to get to know the team, ask questions, and feel confident managing your diabetes as an adult.
When you receive your invitation, please contact us to book your first appointment. During this appointment, we’ll talk through your diabetes care, answer any questions you may have, and help you plan for the future.
