Going home from hospital

If you are to attend an outpatient clinic or day centre after your discharge from hospital, please make sure the appointment has been arranged and that you have a note of the date and time. In some specialties, a convenient appointment will be arranged with you.

If you require transport for your appointment, please check that this has been arranged before you leave the ward. If, however, your appointment is for more than four weeks ahead, ask the Sister for details of how to arrange transport.

The Doctor may decide it is necessary for you to continue taking any medicine he/she has prescribed during your stay. Please ask the pharmacy staff or ward staff, if you are unsure about any of your new medication. If the medicine is for a short time only (such as painkillers or antibiotics) one pack will be given to you. If the medication is to be taken long term, up to 28 days supply of any new medication will be given to you. The medication will be noted on your GP letter and further supplies are obtained from your GP.
