Community Health Project Recognised At National Awards

A local community health project has been recognised at a national awards ceremony. 

The Patient Experience Network (PEN) National Awards celebrate the delivery of outstanding patient experience by health and social care organisations. 

Central Cheshire Integrated Care Partnership (CCICP), which manages community health services in the South Cheshire and Vale Royal areas, were finalists at the recent event. 

The Advanced Community Practitioner (ACP) Rapid Response Service’s team were finalists in the ‘Integration and Continuity of Care’ category. 

This new service is an innovative and alternative approach to hospital admissions for patients in the community. With the support of the patient's own GP, they assist adults who have acute ill-health or an exacerbation of a long term illness, to be treated and cared for at home where appropriate. This has prevented hospital admission in 72 percent of cases so far. 

Laura Lane, Advanced Community Practitioner, said: “It is brilliant that we were shortlisted. The service's success is due to the collaborative working of all community services with primary care; we could not deliver our service without the help and support of the rest of the multi-disciplinary team. 

“We also work closely with our colleagues at Leighton Hospital and if we feel our patients require a hospital admission this enables a smooth patient journey from community to hospital.” 

Joanne Bowen, Head of Quality, Nursing and Professional Lead at CCICP, said: “It’s great to see one of our teams get the recognition they deserve for their continued work to improve patient experience at Central Cheshire Integrated Care Partnership. 

“It’s an accurate reflection on the dedication that our staff across all of CCICP gives to ensure the best in patient experience."
