Our mission is to provide high-quality, connected care across hospital, community and neighbourhood settings. We will achieve this through five models of care that support people throughout their lives, five strategic objectives that focus our efforts where they matter most, and through significant investment in the future of healthcare, including the development of a new Leighton Hospital.

A Healthier Future for Mid Cheshire is our long-term plan for improving health and care across Crewe, Nantwich, Winsford, Northwich and surrounding areas.
It sets out how we will work with local people, staff and partners to deliver high-quality, connected care across hospital, community and neighbourhood settings.
The strategy focuses on prevention, personalised care and partnership - making sure services feel more joined up and easier to access.
It also provides the long-term framework for major investments in our future, including the development of a new Leighton Hospital and modern, digitally connected environments that support outstanding care.
More people are living longer, often with more complex health needs. Demand for services is increasing, and too many people still experience difficulty accessing the right care at the right time.
There are also differences in health outcomes across our communities that we need to address.
This strategy sets out how we will respond to these challenges - improving services while making sure they are sustainable for the future.

Building the future of healthcare for Mid Cheshire
Creating a healthier future for Mid Cheshire is about more than improving services today. It is about building a health and care system that is ready to meet the needs of future generations.
A key part of that future is the development of a new Leighton Hospital.
The new hospital will provide modern, sustainable facilities designed around the needs of patients, families and staff. It will create environments that support high-quality, compassionate care, embrace digital innovation and enable new models of care to flourish. It will also help attract, develop and retain talented colleagues, creating a modern workplace where staff can thrive and deliver outstanding care for future generations.
As healthcare continues to evolve, our hospital must evolve too. The new Leighton Hospital will help us respond to changing population needs, support advances in treatment and technology, and create spaces that are flexible, efficient and fit for the future.
Importantly, the new hospital is not a standalone project. It forms part of a wider transformation of health and care across Mid Cheshire. Alongside stronger neighbourhood services, more care delivered closer to home and greater use of digital technology, it will help create a more connected system that supports people throughout their lives.
Together, these changes will help ensure that patients receive the right care, in the right place, at the right time.
The new Leighton Hospital represents a once-in-a-generation opportunity to improve healthcare, support our workforce, strengthen our communities and create a lasting legacy for future generations.
The strategy has been shaped with staff, patients, carers, communities, partners and stakeholders, ensuring that the ambitions we have set reflect the needs, expectations and aspirations of the people we serve.
We will work alongside local authorities, primary care, community services, voluntary, community and social enterprise organisations, and wider system partners to create a more connected health and care system for Mid Cheshire.
