Reasonable adjustments are small changes that can make a big difference. They help people who need extra support when they come to hospital or go to other health appointments.

Health and care staff must provide reasonable adjustments by law. This is part of the Equality Act 2010.

We will write your reasonable adjustments in your health record. This helps us make sure you get the support you need.

Examples of reasonable adjustments

Interpreters and translation

· Using an interpreter or translation service.

· If you need an interpreter for your appointment, please contact the department you are visiting. They will arrange this support for you.

Communication support

You can tell us how you prefer to communicate. This could be:

· By phone

· By email

· By text message

We can also:

· Use pictures or images to help explain things

· Use technology such as a tablet, phone, or digital pen

· Provide information in other formats, such as Easy Read or Braille

Other support options

We may be able to offer:

· The first or last appointment of the day

· A longer appointment (for example, a double appointment)

· An appointment at a clinic you already know (if available)

· A clinic room with dimmed lights (if available)

· A separate waiting area (if available)

· Flexible visiting, when appropriate

· Someone to come with you to take notes or speak for you. This can be a family member or a professional.
