To become a Member of Mid Cheshire Hospitals NHS Foundation Trust, you can:

• Download a Membership Form (PDF)

• Contact the Membership Office on 01270 612128 to request a Membership Form be issued to you

Membership Forms are also available at clinics across the Trust and in Patient Packs on the Wards.
